The problem
Every diver has to fill in a registration and sign a medical and liability statement before going in the water, and the statement depends on the programme: a first-time Discover Scuba dive asks different questions from a certified fun dive. On paper that meant stacks of forms at the counter in the busiest hours, handwriting to decipher afterwards, and records that could only be found by leafing through a folder.
What we built
Customers open the form on their own phone at the centre, choose their language and programme, and the form shows only the questions and statements that apply, ending in a signature field. Each submission is copied to Google Sheets the moment it is sent, so there is always a plain backup. A separate Laravel and FilamentPHP system holds the registrations with search, a calendar view, and charts of which programmes sell and when the busy periods fall.
